At a temporary worksite located at 585 East Lansdowne Avenue, Orange City, FL 32763: On or about June 26, 2024, the employer exposed employees to eye injury hazards, in that eye protection was not utilized by employees who were operating pneumatic nail guns to nail down roof shingles.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a. On a 4:12 pitch roof of a temporary worksite located at 585 East Lansdowne Avenue, Orange City, FL 32763: On or about June 26, 2024, the employer exposed employees to fall hazards, in that, fall protection was not utilized by employees while performing roofing work.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  a. At the front of the home, near the door of a temporary worksite located at 585 East Lansdowne Avenue, Orange City, FL 32763: On or about June 26, 2024, the employer exposed employees to fall hazards in that, the side rails of a portable ladder used to gain access to the roof, did not extend three feet above the roof, exposing employees to a fall hazard.
