29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person: (a) 2 Ironhorse Trail, Durango, CO 81301: On or about June 25, 2024, and at times prior, the creating, correcting, and exposing employer did not ensure that subcontractor employees performed tasks including installing siding and soffits from a mobile scaffolding platform which was designed by a qualified person. This condition exposed employees to a fall hazard in excess of 17 feet.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: (a) 2 Ironhorse Trail, Durango, CO 81301: On or about June 25, 2024, and at times prior, a Terex Handler [SN 7799] was used to support scaffold platforms used by employees to install siding and soffit. The creating, exposing, and correcting employer did not ensure that the Terex Handler was designed to support scaffold platforms. This condition exposed employees to a fall hazard in excess of 16 feet and crushed-by hazards.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: (a) 2 Ironhorse Trail, Durango, CO 81301: On or about June 25, 2024, and at times prior, the creating, exposing, and correcting employer did not ensure that the job-built scaffolding platform attached to a Terex Handler [SN 7799] all-terrain forklift was fully supported. This condition exposed subcontractor employees to a fall hazard in excess of 16 feet and crushed-by hazards.
