29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: (a) Commercial building rooftop, various locations; On or about June 27, 2024, and dates prior, employees working from a low-slope roof were not protected from falls while engaged in roofing system replacement activities at approximate heights ranging between 12-foot, 9-inches to 20-feet, 9 inches above ground-level.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) Commercial building, lower roof; On or about June 27, 2024, and dates prior, the side rails of a Werner model D1220-2 aluminum extension ladder, used by employees to gain access to the lower level rooftop, did not extend at least 3-feet above the landing surface, was not secured to prevent displacement, and grab handles were not provided. (b) Commercial building, upper roof; On or about June 27, 2024, and dates prior, the side rails of a multi-position aluminum ladder, set-up as an extension ladder, and used by employees to gain access to the upper level rooftop, did not extend at least 3-feet above the landing surface, was not secured to prevent displacement, and grab handles were not provided.
