29 CFR  1926.602(c)(1)(vii):Unauthorized persons were allowed to ride on a powered industrial truck:  a.) On or about 06/12/2024, at 22511 Stearman Way, Tanner, AL; the employer exposed employees to fall hazards, in that employees were riding in a basket attached to the forks of the powered industrial truck.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.760(a)(1):Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  a.) On or about 06/12/2024, at 22511 Stearman Way, Tanner, AL; the employer exposed employees to fall hazards in that employees were not required to wear fall protection when erecting a steel frame structure at heights of approximately 30-feet.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

On or about 06/12/2024, at 22511 Stearman Way, Tanner, AL; the employer exposed employees to fall hazards, in that employees were not trained to recognize hazards with erecting a steel frame structure at heights of approximately 30-feet.
