29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: Location: 45 Mount Prospect Ave. Newark NJ 07104 a: The employer failed to provide safe means of egress from a trench that was more than 4-ft deep. An employee engaged in excavation activities was exposed to injuries associated with cave-in for not being provided with safe means of egress from an 11-foot deep trench. Violation occurred on or about July 8, 2024.

Location: 45 Mount Prospect Ave. Newark NJ 07104 a: The employer failed to implement an adequate protective system during excavation work. An employee working in a 11-ft deep trench was exposed to hazards associated with cave-in for not being provided with adequate protective systems that include but not limited to a trench box. Violation occurred on or about July 8, 2024.
