29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:   Location: Second story roof  a) On or about 7/11/24, the employer did not ensure that employees working on a roof greater than 17 feet above the ground were protected against injuries received from improper use of a personal fall arrest system. Employees installing wood roofing shingles were wearing personal fall arrest harnesses, however, the employees were connected to the roof by the front of the harness, and not the center of the wearer's back near shoulder level.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Location: Second story roof  a) On or about 7/11/24, the employer did not ensure that employees working on a roof greater than 17 feet above the ground were protected against falls. Employees installing wood roofing shingles were wearing personal fall arrest harnesses, however, the employees were not connected to the roof.  Erect Construction Corporation was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1538183, citation number 1, item number 1 and was affirmed as a final order on December 21st, 2021, with respect to a workplace located at 658 Andover Street, Lowell, MA 01852.
