29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Employee(s) are exposed to fall hazards while working on a residential roof. This was most recently documented on or about July 11, 2024 at a residential house located at or about 16533 Ames Avenue, Omaha, Nebraska. Employee(s) were working on a roof measured at about 10 feet with a slope of about 45 degrees without fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Employee(s) are exposed fall hazards when ingress and egress a residential roof. This was most recently documented on or about July 11, 2024 at a residential house located at or about 16533 Ames Avenue, Omaha, Nebraska. A portable ladder used for ingress and egress the roof did not extend above the upper landing surface by at least 3 feet.  A) Ladder on the north side of the roof was not extended 3 feet above the landing surface B) Ladder on the west side of the roof was not extended 3 feet above the landing surface
