29 CFR 1910.28(b)(2)(i): The employer did not ensure that each employee in a hoist area was protected from falling 4 feet (1.2 m) or more to a lower level by a guardrail system, a personal fall arrest system, or a travel restraint system: The employer is failing to protect employees from fall hazards. This was most recently documented on or about July 9, 2024, when the employer did not ensure that the swing gate was closed after loading empty pallets into the loft, and prior to operating the Yale pallet jack to move the empty pallets. Employee fell from the loft/mezzanine to a concrete floor below, exposed to 10 feet fall hazards.

29 CFR 1910.305(a)(2)(ix): Lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lamp holder with a guard: Employees engaged in loading, unloading materials at the loft area by the stair were exposed to burns and laceration hazards in that the fluorescent lights were not covered with protective devices in the event of accidental contact with the lights. The fluorescent light was measured 5 feet 9 inches from the floor.
