29 CFR 1926.20(b)(2): The employer did not ensure that a program of frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons was in use:    a) Job site, 1805 Morrell Street, Pittsburgh, PA: On or about July 18, 2024, during roofing activities, the employer did not ensure that a system was in place to identify and correct unsafe working conditions. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection, not using personal protective like safety glasses or hardhats, wearing normal shoes around the site where they could step on nails and failing to ensure that ladders had proper labeling, were not set up crooked or that they extended at least 3 feet above the landing areas.
