29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the sequence listed.   On or about July 17, 2024, the employer did not ensure that maintenance employees implemented energy control application steps prior to allowing them to perform adjustments and/or repairs after troubleshooting issues on equipment, to include the Ferrit Microwave and conveyors, in the bakery department. Employees were exposed to electrical shock related injuries which could result in permanent disability or death.   As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:  On or about July 17, 2024, the employer did not ensure affected maintenance employees were provided the required electrical protective equipment for working on or around energized electrical conductors or circuit parts in excess of 240V, such as but not limited to non-conductive head protection, FR clothing, and protection against Arc Flash hazards. Employees are exposed to electrical shock related injuries that could result in severe burns, organ/tissue damage, or death.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
