(a) On or about July 19, 2024, at a jobsite located at 10777 S Jessica Dr, Oak Creek, WI 53154, the employer did not perform frequent and regular inspections of the job site to ensure that hazardous conditions were identified and corrected.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about July 19, 2024, at a jobsite located at 10777 S Jessica Dr, Oak Creek, WI 53154, employees installing a roof on an existing residential home with an approximately 9:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 17 feet 1 inch from the roof eve to the ground below on the back (West) side of the home.
