29 CFR  1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).  a.  On or about June 5, 2024, an employee performed disassembly and cleaning of a Hobart meat grinder while the equipment was energized and operational. The employer did not ensure that the grinder was disconnected from its electrical energy source prior to performing the servicing activity, thereby exposing the employee to caught-in injuries hazards created by from the grinder's rotating auger.  b.  On or about June 5, 2024, an employee performed disassembly and cleaning of a Hobart meat saw while the equipment was energized and operational. The employer did not ensure that the saw was disconnected from its electrical energy source prior to performing the servicing activity, thereby exposing the employee to caught-in hazards created by the saw's blade.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a.  On or about June 6, 2024, the employer failed to report the amputation of an employee's right thumb in the course of routine cleaning duties to OSHA within the required 24 hours.

29 CFR  1904.41(a)(1)(i): If your establishment had 20-249 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.  a.  On or about July 24, 2024, the employer failed to submit their CY 2023 OSHA Form 300A data through the Injury Tracking Application (ITA) by March 2, 2024.
