29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about August 7, 2024, in the manufacturing area where employees were using a  Schauer Machine brand high-speed lathe without a guard to cover the rotating chuck.

29 CFR 1910.1025 (h)(1): All surfaces were not maintained as free as practicable of accumulations of lead:  On or about August 7, 2024:  a.	In the breakroom area where Luch Table #1 was wipe sampled and found to have a 5.95 Microgram accumulation of lead in the 10cm square area tested.  b.	In the breakroom area where Luch Table #2 was wipe sampled and found to have a 7.093 Microgram accumulation of lead in the 10cm square area tested.  c.	In the shipping area where Shipping Table #1 was wipe sampled and found to have a 1.270 Microgram accumulation of lead in the 10cm square area tested.  d.	In the inspection area where Inspection Table #1 was wipe sampled and found to have a 8.379 Microgram accumulation of lead in the 10cm square area tested.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  On or about August 14, 2024, throughout the facility where employees were exposed to eight hour time weighted averages of 86.5 decibels before an effective hearing conservation program had been implemented.
