1910.1200(h)(3)(i): The employee training did not include the methods and observations used to detect the presence or release of a hazardous chemical in the work area:   Employees were exposed to chemical hazards associated with carbon dioxide in the ground beef area. This was most recently documented on July 31, 2024, in the ground beef area, located at 2000 E. Trail St. Dodge City, Kansas. Employees were exposed to carbon dioxide released by ground beef blenders when employees were exposed to levels in excess of 5,000 ppm.  1 employee working in the ground beef top side area was exposed to an 8-hour time weighted average (TWA) level of 5,751 ppm, which is 1.5 times the OSHA permissible exposure level (PEL) of 5,000 ppm.

1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used:  On or about July 31, 2024, located at 2000 E Trail St. Dodge City, Kansas, employees were exposed to chemical hazards associated with carbon dioxide in the ground beef area. Employees were exposed to respiratory hazards from carbon dioxide exposure above the Permissible Exposure Limit without the use of effective administrative or engineering controls in place.
