29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a) Newman Hall Catholic Center, 8 Calvert Street, Cortland, NY  13045, on the roof, on or about 7/23/24:  Workers were performing roofing work on a metal 4:12 pitch roof with a ground to eave height of 15 feet from ground without fall protection of any kind.

29 CFR  1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):   a) Newman Hall Catholic Center, 8 Calvert Street, Cortland, NY  13045, on the roof, on or about 7/23/24:  Workers were performing roofing work on a metal 4:12 pitch roof with a ground to eave height of 15 feet from ground without fall protection of any kind and without demonstrated competency with regard to using fall protection systems by the employer.  Abatement certification must be submitted for this item.

29 CFR  1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause them to lose balance and fall:  a) Newman Hall Catholic Center, 8 Calvert Street, Cortland, NY  13045, on or about 7/23/24:  Workers were installing and replacing soffit panels from ladders, 26 feet from ground in a way that required them to remove both hands to perform the work; holding the material, overhead, with one hand, and a drill-gun with the other hand.
