(a) On or about July 30, 2024, the employer did not ensure that an employee preforming service and maintenance covered activities on an Accurpress 71008 Advantage Press Brake was provided with authorized lockout/tagout training prior to performing servicing and maintenance activities, exposing employee(s) to mechanical crushing hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about July 30, 2024, located in the fabrication area F7, a Fabbrica Macchine Bergamo Manual Triton Band Saw had a section of its blade exposed that was outside of the working portion of the blade, exposing employees to an amputation hazard.
