29 CFR  1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.   Location: 234 East Main Street, East Islip, NY 11730.  On or before 07/30/2024, The employees were not provided with a means of access to a scaffolding platform approximately 10ft above the ground.      Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR  1926.451(g)(1)(vii):For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  Location: 234 East Main Street, East Islip, NY 11730.  On or about 07/30/2024, Employees were installing siding on a commercial store front while working from a scaffolding platform which was supported by two A Frame ladders.  Employees were not protected from falling approximately 10ft to the ground below.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
