29 CFR  1910.28(b)(6)(ii): The employer did not ensure that each employee 4 feet (1.2 m) or more above dangerous equipment must be protected from falling by one of the systems listed in 29 CFR 1910.28(b)(6)(ii)(A)-(D).  On or about July 30, 2024, at the shop building, fall protection was not used to protect employees from falling into the hopper for the trash compactor which was greater than 4 feet below the floor.

29 CFR  1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail as required by the regulation:   On or about June 17, 2024, and at times thereafter, the employer's 300 log for year 2024 was not completed with the required detail in that the number of days away from work were not entered for a June 17, 2024 work-related recordable injury in which the employee fell from a refuse collection truck and received in-patient hospitalization.

On or about July 31, 2024, the employer did not provide to OSHA, within the four business hours of when they were requested, the OSHA Form 301 Injury and Illness Incident Report, or equivalent for a recordable work-related injury that occurred June 17, 2024, in which an employee fell from a refuse collection truck and was hospitalized as an in-patient.

29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition:  On or about July 30, 2024, outside of the office, employees used an exterior stairway in which the lowest stair tread was slanted and was not maintained in a safe condition.
