29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about July 31, 2024, at the job-site located at 22928 Yarn CT Land O' Lakes Florida; employees were exposed to a 10-foot fall hazard while installing roof shingles on 4 in 12 pitched (vertical to horizontal) roof, in that, fall protection was not used.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:  (a) On or about 7/30/2024,  At 22928 Yarn CT Land O' Lakes Florida; employees involved in residential roof work were exposed to a 10 foot fall hazard, in that, the multi-position ladders were not extended 3 feet above the landing surface of the roof.
