29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge 6 feet (1.8 m) or more above a lower level was not protected from falling by use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about July 18, 2024, an employee working for Sheet Metal Contractors Inc. was working on a covered walkway at an elementary school building located at 1044 Highschool Drive in Saint Clair, Missouri. The employer did not ensure that fall protection systems were utilized by the employees working approximately 9 feet above the lower level.  An employee was hospitalized when falling to the ground below.

29 CFR 1904.39(a)(1): The employer did not report an in-patient hospitalization of an employee resulting from a work-related incident within 24 hours:  On or about July 18, 2024, an employee working for Sheet Metal Contractors Inc. was injured when falling to the ground from an elevated working surface at an elementary school building located at 1044 Highschool Drive in Saint Clair, Missouri.  The employer failed to report the in-patient hospitalization of the employee to OSHA within the twenty-four (24) hour reporting requirement. OSHA was not notified until July 23, 2024, outside of the 24-hour reporting requirement.
