On or about July 30, 2024, at the above addressed jobsite, an employee using a pneumatic nail gun was not wearing safety glasses, thereby exposing the employee to hazards associated with eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a. On or about July 30, 2024, at the above addressed jobsite, employees performing residential roofing operations, nine (9') feet one (1") inch above the ground, were not protected from falls, thereby exposing employees to fall hazards.  b. On or about July 30, 2024, at the above addressed jobsite, an employee performing residential roofing operations, seventeen (17') feet eight (8") inches above the ground, was not protected from falls, thereby exposing employee to fall hazards.

On or about July 30, 2024, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
