29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented.  On or about August 1, 2024, on the front right side of the roof, employees were performing roofing operations at a height more than six feet above the ground without the use of  fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access.  On or about August 1, 2024, on the front right side of the roof, employees used a ladder to gain access to the roof without being extended at a minimum of three feet above the upper landing surface.
