29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   Worksite located at 1431-1433 SW Winthrop Terrace, Lee's Summit, Missouri- Employees were exposed to fall hazards while working on a steep residential roof and were not protected from falling by the use of any means of fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:   Worksite located at 1431-1433 SW Winthrop Ave., Lee's Summit, Missouri- Employees were exposed to fall hazards in that the ladder being used to access the upper landing of the roof was not raised at least 3 feet above the upper landing surface.
