29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin severe cuts or lacerations:  Grind area of warehouse - On or about July 18, 2024, employees were exposed to laceration and amputation hazards where cut resistant gloves were not provided or required to be worn during cleaning and maintenance activities on the Chamberland 12" granulator.

Grind area of warehouse - On or about July 18, 2024, an employee sustained an amputation of three fingers and the employer did not report the amputation to OSHA within 24 hours of the incident.
