At a residential worksite located at 9627 N Lucerne Ave KC MO -- employees engaged in roofing duties using pneumatic nail guns and other small hand tools were not wearing any eye protection. This exposed the employees to getting shrapnel and debris in their eyes causing injury. This was documented on or about 8/7/2024.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At a residential worksite located at 9627 N Lucerne Ave KC MO -- employees engaged in roofing duties were using two extension ladders. One was for access to the lower flat roof. The second was from the flat roof to the steep roof. The second ladder did not extend 3 feet above the landing surface -- the roof being the landing surface. This exposed the employees to fall hazards. This was documented on or about 8/7/2024.
