Location - Throughout.  On or about 8/13/2024, employees were not trained prior to being permitted to operate the Global electric pallet jack and the Clark Equipment Power Worker ("pallet stacker") powered industrial trucks.

Location - Throughout.  On or about 8/13/2024, the employer did not maintain OSHA 300 logs or equivalent documentation which listed each work-related fatality, injury, or illness case as required by the OSHA standard.
