29 CFR 1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) a) At 1408-1442 SR-49, Tuskegee, AL; On or about August 9, 2024 and at times prior, the employer exposed employees to crush-by and struck-by hazards in that the employer failed to conduct frequent visits and inspections to the job sites to ensure employees were trained to operate heavy equipment such as but not limited to a Caterpillar 279D3 P.I.N. *CAT0279DARB904057* Skid Steer and a Caterpillar Mulching head.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) At 1408-1442 SR-49, Tuskegee, AL; On or about August 9, 2024 and at times prior, the employer exposed employees to crush-by and struck-by hazards in that the employer failed to instruct employees in the recognition and avoidance of unsafe conditions applicable to their work environment. Employees were allowed to operate a Caterpillar 279D3 P.I.N. *CAT0279DARB904057* Skid Steer and a Caterpillar Mulching head without first being trained in operation of these machines.
