On or about August 14, 2024, at the above addressed jobsite, an employee was exposed to slip, trip and fall hazards when the ladder used to access the ground had a shingle hoist attached, there-by exposing employee to fall hazards of twenty-one (21) feet from hoist on the bottom of the ladder.

On or about August 14, 2024, at the above addressed jobsite, employees using pneumatic nail guns were not wearing safety glasses, thereby exposing employees to hazards associated with eye injuries.  Bermejo Remodeling Inc. dba Angel Pina was previously cited for a violation of this Occupational Safety and Health Standard 1926.102(a)(1) which was contained in OSHA inspection number 1597942, citation number 1 item number 2 and was affirmed as a final order on September 9, 2022, with respect to a workplace located at 729 S Oak Park Ave, Oak Park, IL.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about August 14, 2024, at the above addressed jobsite, an employee performing residential roofing operations, nineteen (19) feet three (3) inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  b.  On or about August 14, 2024, at the above addressed jobsite, employees performing residential roofing operations, twenty-three (23) feet seven (7) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  c.   On or about August 14, 2024, at the above addressed jobsite, an employee performing residential roofing operations, twenty (20) feet above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  d.   On or about August 14, 2024, at the above addressed jobsite, an employee performing residential roofing operations, twenty-three (23) feet six (6) inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.
