29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a.) Central States Commercial Roofing LLC. 1618 Ash St. Erie, PA 16503: Commercial roof- on or about 08/16/2024 and at times prior thereto; employees doing roof work operations at the edge of the structure on a commercial flat roof were not protected from falls of 24-feet to the ground level. Employees were carrying roof materials that include but not limited to insulated roofing panels along the edge of the roof.

29 CFR  1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines:   a.) Central States Commercial Roofing LLC. 1618 Ash St. Erie, PA 16503: Commercial roof- on or about 08/16/2024 and at times prior thereto; employees doing roof work operations at the edge of the structure on a commercial flat roof were not protected from falls of 24-feet to the ground level. The access point used by employees to access the roof with the scaffold tower did not have an access path formed by two warning lines.
