Employees conducting residential framing work were observed passing materials from the ground level to the roof without wearing protective helmets.

Employees conducting residential framing work were observed carrying materials to the roof top by utilizing portable ladders. The portable ladder side rails did not extend at least 3 feet (.9 m) past the upper landing surface being accessed.
