29 CFR  1926.1053(b)(5)(i):Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder:   At a new multi-family residential worksite located at Indiana Ave & NE 87th St KC MO -- employees engaged in roofing duties were using an extension ladder to gain access to the roof. The ladder was not at the 4:1 ratio maintaining an approximate 75 degree angle. This exposed the employees to fall hazards in that the ladder could have slipped and fallen causing serious injury or death. This was documented on or about 8/20/2024.

29 CFR  1926.102(a): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  At a new residential worksite located at N Indiana Ave & NE 87th St KC MO -- employees engaged in roofing duties using pneumatic nail guns and other small hand tools were not wearing eye protection. This exposed the employees to shrapnel and debris to get in their eyes. This was documented on or about 8/20/2024.   Gustavo Lopez Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1664971, citation number 1, item number 2 and was affirmed as a final order on 8/17/2023, with respect to a workplace located at 10121 N Kansas Ct KC MO.  Gustavo Lopez Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1657045, citation number 1, item number 1 and was affirmed as a final order on 3/15/2023, with respect to a workplace located at 12802 S Foxborough Ct., Lees Summit MO.

This exposed the employees to fall hazards causing serious injury or death. This was documented on or about 8/20/2024.  Gustavo Lopez Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1664971, citation number 1, item number 3 and was affirmed as a final order on 8/17/2023, with respect to a workplace located at 10121 N Kansas Ct KC MO. The ladder did not extend 3 feet above the landing surface. This exposed the employees to fall hazards causing serious injury or death. This was documented on or about 8/20/2024.
