29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  On or about August 22, 2024, Employees engaged in residential roofing activities at 6602 Harvest Dr, Papillion, Nebraska were exposed to fall hazards in that the employees were not using any form of fall protection while working from a height of 10 feet above the ground.

29 CFR  1926.1053(b)(6): Ladders were used on surfaces which were not stable and level:   Employees engaged in residential roofing activities at 6602 Harvest Dr, Papillion, Nebraska were exposed to fall hazards in that they used a ladder with one footing on grass and one footing on the concrete driveway making the ladder uneven and unstable.
