29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  On or about August 19, 2024, the exposing employer, Jaime Roofing LLC, did not ensure the rails of a portable ladder extended at least 3 feet beyond the first story of the residential roof.  The ladder was placed on the ground and employees engaged in residential roofing operations were using it to access the first story of the roof, which measured approximately 10 feet.  This condition exposed employees to fall hazards.

29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a) On or about August 19, 2024, the exposing employer, Jaime Roofing LLC, did not ensure employees were protected from fall hazards of approximately 18 feet while engaged in residential roofing operations.  This condition exposed employees to fall hazards.  Jaime's Roofing, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA Inspection 1551852, Citation 1, Item 1, which was affirmed as a final order on December 14, 2021, with respect to the workplace located at 1. Parker, CO 80134.
