29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):                                                                                                    Employee(s) were exposed to electrical hazards. This was observed on or about August 28, 2024, at 1821 Bacon Drive, Gothenburg, Nebraska. Employees were not provided with an energy control program by the employer that met the requirements of 1910.147, including, but not limited to; developing specific energy control procedure for the Air Compressor.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet:                Employee(s) were exposed to fire hazards and concussion injuries. This was observed on or about August 28, 2024, at 1821 Bacon Drive, Gothenburg, Nebraska.  Six acetylene compressed gas cylinders were stored  on the Southwest Wall less than twenty feet apart from 5 oxygen gas cylinders.

This was observed on or about August 28, 2024, at 1821 Bacon Drive, Gothenburg, Nebraska. The employer is using a daisy chain method in the form of a drop cord, plugged in by a relocatable power tap that was not being used in accordance to its' Underwriters Laboratories listing to power a Master Force 30" Heavy-Duty drum fan in the Weld Bay.
