29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a.) Dereume Construction Group LLC DBA Dereume Dunkel Roofing Company, 320 Liberty Blvd., Dubois, PA 15801: Commercial roof- on or about 08/28/2024 and at times prior thereto; employees were performing roof work operations outside the warning line system. Employees were exposed to fall hazards of approximately 14-feet above ground level.

Dereume Construction Group LLC DBA Dereume Dunkel Roofing Company, 320 Liberty Blvd., Dubois, PA 15801: Commercial roof- on or about 08/28/2024 and at times prior thereto; employees were performing roof work operations outside the warning line system and were exposed to fall hazards of approximately 14-feet above ground level. The safety monitor was performing tasks distracting him from the monitoring function.

29 CFR  1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines:   a.) Dereume Construction Group LLC DBA Dereume Dunkel Roofing Company, 320 Liberty Blvd., Dubois, PA 15801: Commercial roof- on or about 08/28/2024 and at times prior thereto; employees were performing roof work operations with an incomplete warning line system. Employees were exposed to fall hazards of approximately 17-feet above ground level. The access point used by employees to access the roof with the Werner extension ladder did not have an access path formed by two warning lines.
