Location: Job site;  On or about 08/29/2024, employees were allowed to utilize pneumatic nail gun(s) without proper eye/face protection to protect against potential flying objects.

Location: Job site;  On or about 08/29/2024, the foreman and two employees were allowed to engage in roofing activities without the use of fall protection on a residential roof that was 19-feet or more above the ground below.

Location: Job site;  On or about 08/29/2024, multiple employees were observed incorrectly wearing their fall protection harnesses while working 19-feet above the ground below from atop residential roof and were tied off to other carabiners or attachment points on their harnesses.
