29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about August 16, 2024, and at times prior thereto, roofers were performing reroofing activities on the roof of an existing residential house at heights above 6 feet without the use of fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about August 16, 2024, and at times prior thereto, the employer did not certify that each employee exposed to a fall hazard above 6 feet had been trained on a fall protection program.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On or about August 16, 2024, a roofer fell from a roof resulting in injuries requiring hospitalization for approximately 6 days. The Employer did not notify the local OSHA area office of the hospitalization within 24-hours.
