29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) The Rob Salamida Co., Inc., 71 Pratt Avenue, Johnson City, NY - Dry Batch. On or about September 3, 2024, employees performed servicing activities on equipment including, but not limited to, the Ribbon Blender without ensuring the equipment was inoperable while cleaning its internal surfaces and auger. The equipment was not locked out during servicing. Procedures were not documented and utilized.

29 CFR  1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips, and sparks:  a) The Rob Salamida Co., Inc., 71 Pratt Avenue, Johnson City, NY - Production Area. On or about September 3, 2024, the Line 1 Capper was not guarded to protect workers from rotating parts. The employer had not ensured rotating parts were guarded.    b) The Rob Salamida Co., Inc., 71 Pratt Avenue, Johnson City, NY - Production Area. On or about September 3, 2024, the Line 1 Label Stamp was not guarded to protect workers from the point of operation. The employer had not ensured the point of operation was guarded.
