29 CFR  1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agent.  (a) On or about September 4, 2024, at 1425 Manistique Ave, South Milwaukee, WI 53172, the employer did not ensure employees conducting installation of a new residential roof, using hand tools and nail guns were wearing eye protection. This exposed the employees to struck by hazards and eye injuries.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about September 4, 2024, at 1425 Manistique Ave, South Milwaukee, WI 53172, employees installing a roof on an existing residential home roof with an approximately 10:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 10 feet 2 inches from the roof eve to the ground below on the back (south) side of the home.  (b) On or about September 4, 2024, at 1425 Manistique Ave, South Milwaukee, WI 53172, employees installing a roof on an existing residential garage with an approximately 5:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 7 feet 2 inches from the roof eve to the ground below on the right (south) side of the garage.
