29 CFR  1926.20(b)(4): The employer did not ensure that the employee(s) required to operate equipment and machinery was qualified by training or experience.  Chrysalis Townhouse construction development, 834 Main Street, New Britain, CT: On or about September 4, 2024, a Caterpillar CS56 S/N:C5S01272 tipped over into a hole due to operating near/or on the edge of the hole without proper supervision while the operator in training was performing soil compacting activities exposing them to struck against hazards. Job site organization was not maintained in accordance with the Caterpillar CS56 Operation and Maintenance manual.
