29 CFR 1926.451(b)(3): The front edge of two working platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling.  Location: Job site;  On or about 09//2024, employees were allowed to work in an area of scaffolding that was approximately 16-18-inches away from the face of the work at the corner of the face of work where the scaffolding changed directions and created a gap large enough for an employee to fall 7-feet to the platform below.
