29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a)  On or about September 6, 2024, the employer did not ensure employees were protected by a protective helmet.  Employees were exposed to a possible danger of head injury from impact or from falling or flying objects, while working below employees installing roofing material from the second story roof of an existing residential property.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a)  On or about September 6, 2024, the employer did not ensure that employees using pneumatic nail guns were wearing eye protection equipment, exposing the employees eyes to a struck-by and impalement hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about September 6, 2024, employees conducting roofing work on a new residential property with a 6:12 pitch roof, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 17 feet to the ground below.

29 CFR  1926.503(c)(3): 29 CFR 1926.503(c)(3): When inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill, the employer shall retrain the affected employee.  (a)  On or about September, 6, 2024, the employer did not ensure employees exposed to potential fall hazards had been properly trained to recognize and minimize the fall hazards associated with their work, and how to properly install anchor points and ensure proper selection of fall protection equipment.
