29 CFR  1910.132(d)(2):The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done:  On or about September 06 2024, and at times prior thereto, employees performed under pressure tank cleaning operations to a beer brewing tank with nitrile gloves and chemical goggles, and without a written and certified hazard assessment performed by the employer of the operation and personal protective equipment used by the employees.

29 CFR  1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:  On or about September 6, 2024, and at times prior thereto, employees performed cleaning under pressure to tanks with a corrosive cleaner without ensuring the tank outlet valves were accurately tagged out to prevent unexpected release of the corrosive cleaning solution.

On or about September 6, 2024, and at times prior thereto, employees performed tank cleaning procedures to tanks under pressure with a corrosive cleaner without effective training to ensure and prevent an unexpected release of the corrosives in the tank.
