On or about September 9, 2024, employees were exposed to fall hazards approximately 9 feet, 4 inches high while performing roofing operations on a residential property.  Employees were observed not using fall protection correctly when the lifeline lanyard was connected to the front nylon straps while performing roofing operations.

On or about September 9, 2024, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface nor was it secured at the top, thereby exposing employees to the hazards associated with falls.
