29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section: a) 9000 River Rd, Pennsauken, NJ - Extruder J9 - The employer allowed maintenance personnel and operators to access the operating extruder through a gate/fence surrounding the extruder without implementing Lock out/Tagout procedures thus exposing employees to struck-by and caught-in-between hazards. This condition was observed on or about September 10, 2024. b) 9000 River Rd, Pennsauken, NJ - Extruder J7 - The employer allowed maintenance personnel and operators to access the operating extruder through a gate/fence surrounding the extruder without implementing Lock out/Tagout procedures thus exposing employees to struck-by and caught-in-between hazards. This condition was observed on or about September 10, 2024. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) 9000 River Rd, Pennsauken, NJ - Extruder J9 - The electronic safety device for the fence/gate surrounding Extruder J9 was disconnected. The employer allowed maintenance personnel and operators to access the operating extruder through a gate/fence surrounding the extruder thus exposing employees to struck-by and caught-in-between hazards. This condition was observed on or about September 10, 2024. b) 9000 River Rd, Pennsauken, NJ - Extruder J7 - The electronic safety device for the fence/gate surrounding Extruder J7 did not shut down the operation, the puller was still in operation. The employer allowed maintenance personnel and operators to access the operating extruder through a gate/fence surrounding the extruder thus exposing employees to struck-by and caught-in-between hazards. This condition was observed on or about September 10, 2024. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Employees perform servicing and maintenance activities on equipment including, but not limited to Extruders J9 and J7, which has Hydraulic and electric as its energy source. Noted on or about November 6, 2024. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
