The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. 16 West Road, Enfield, Connecticut roofing project: On or about September 4, 2024, employees were connected to the D-ring on the side of a body belt-style harness with the lanyard and rope assembly as an attachment point instead of the D-ring on the upper back of the harness while performing roofing work activities exposing them to a fall hazard of approximately 20 feet from the eave of the roof to the ground on a 8/12 pitch roof.
