29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) On or about September 11, 2024, at a jobsite located at 1820 Kristen Lane, South Milwaukee, WI 53172, the portable ladder being used by employees to access the roof of the structure at or near the front door did not extend at least three feet above the upper point of contact with the eave nor was it secured in place. This exposed employees accessing and egressing the roof, to a fall hazard of approximately 8.7 feet onto the grass covered ground.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about September 11, 2024, at a jobsite located at 1820 Kristen Lane, South Milwaukee, WI 53172, employees conducting roofing activities (shingle installation) on a 4:12 pitch roof of the structure were not protected from falling to the lower level. This exposed employees to a fall hazard from the first floor roof of approximately 8.7 feet at the eave to 14.6 feet at the ridge. (b) On or about September 11, 2024, at a jobsite located at 1820 Kristen Lane, South Milwaukee, WI 53172, employees conducting roofing activities (shingle installation) on a 4:12 pitch roof of the detached garage structure located on the jobsite were not protected from falling to the lower level. This exposed employees to a fall hazard from the garage roof of approximately 8.7 feet.
