Josias Emmanuel Granados Martinez LLC, located at the job site 130 Johnson Drive, Delaware, OH 43015: On or about September 13, 2024, the employer did not initiate and maintain safety and health programs to protect employees performing residential framing construction work including, but not limited to, fall protection/prevention, heat stress, ladder safety, and personal protective equipment. As a result, employees performing residential roofing work were exposed to falls, heat illness, and struck-by hazards.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502: a. Josias Emmanuel Granados Martinez LLC, located at 130 Johnson Drive, Delaware, OH 43015: On or about September 13, 2024, the employer did not ensure employees engaged in residential roofing activities were protected from falls, in that an employee was installing sub-roofing material and securing plywood at the unprotected edge of the back eave of a two-story new build residential home without fall protection measures in use. The employee was thereby exposed to fall hazards of up to approximately 19 feet. b. Josias Emmanuel Granados Martinez LLC, located at 130 Johnson Drive, Delaware, OH 43015: On or about September 13, 2024, the employer did not ensure employees engaged in residential roofing activities were protected from falls, in that employees were climbing the front garage eave and ridge of the two-story new build residential home without fall protection measures in use. The employees were thereby exposed to fall hazards of up to approximately 19 feet. c. Josias Emmanuel Granados Martinez LLC, located at 130 Johnson Drive, Delaware, OH 43015: On or about September 13, 2024, the employer did not ensure employees engaged in residential roofing activities were protected from falls, in that employees were installing sub-roof material on the main roof ridge of the two-story new build residential home without fall protection measures in use. The employees were thereby exposed to fall hazards of up to approximately 19 feet.
