29 CFR  1903.19(d)(1):The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a) Citation 1, Item 1, a serious violation of 29 CFR 1926.501(b)(13) with an abatement date of February 20, 2025. (b) Citation 1, Item 2, a serious violation of 29 CFR 1926.503(a)(1) with an abatement date of March 12, 2025.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERFICATIONS OF THE CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about September 17, 2024, at a jobsite located at 1110 East Stonewood Drive, Oak Creek, WI 53154, employees conducting roofing activities (shingle installation) on a 7:12 pitch roof of the residential structure were not protected from falling to the lower level. This exposed employees to a fall hazard from the roof of the structure of approximately 9.2 feet.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about September 17, 2024, at a jobsite located at 1110 East Stonewood Drive, Oak Creek, WI 53154, the employer did not ensure that employees exposed to potential fall hazards had been properly trained to recognize, minimize, or eliminate the fall hazards associated with this work. This exposed employees to a fall hazard  from approximately 9.2 feet onto grass, building materials, and concrete.
