At the job site located at 22625 W 49th Street, Shawnee, Kansas, an employee engaged in residential roofing clean up on the ground level, was exposed to struck by head injuries from falling objects in that head protection was not utilized.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  At the job site located at 22625 W 49th Street, Shawnee, Kansas, employees engaged in residential roofing activities were exposed to fall hazards of approximately 12 feet from the roof's eave to the ground. The employer did not ensure that employees were adequately protected from falling by utilizing guardrail systems, safety net system, or personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  At the job site located at 22625 W 49th Street, Shawnee, Kansas, employees using the portable extension ladder to access the roof to perform roofing activities were exposed to fall hazards of approximately 12 feet from the roof to the ground in that the side rails did not extend at least 3 feet above the landing surface of the roof.  The side rails extended approximately 2 feet.
